Program Description

The quit smoking clinic at the Haldimand War Memorial Hospital is an outpatient clinic that provides a range of services for those who are thinking about quitting or are ready to quit smoking. These services include evidence based tobacco/nicotine assessment, individualized plan of care, and follow up visits to provide support and promotion of self-care management. Our quit program is based on 12 to 26 weeks depending on need and is offered as individual counselling or group programming. We collaborate/work with various community partners and the clinic is available to all members of the community by self-referral or referral by any health or social service provider including other addictions agencies.
