Program Description

Choices for Change in partnership with the Canadian Mental Health Association offers addiction supportive housing. The goal of this program is to increase the health and social outcomes of people with problematic substance use issues by providing stable housing and individual support. The five objectives of this goal are: 1. Provide individual addiction counselling services and support for personal goals; 2. Increase successful tenancy; 3. Reduce frequency of repeated use of addiction treatment, emergency department, and acute care services; 4. Reduce contact with criminal justice system; and 5. Advocate and support individual needs.

Access Information

Applicants will be screened to ensure primary eligibility objectives are met. Primary eligibility requirements are as follows: current problematic substance use and history of multiple entries in treatment continuum; homeless, at risk of being homeless, or marginally housed (current and historical record); unable to obtain or maintain housing without support; willingness to identify harm reduction or abstinence substance use goal(s); capability and willingness to constructively participate in ASH program; and no serious and persistent mental illness or acquired brain Injury which significantly impacts insight and judgment.
